Kind of both and adjust as needed. Although I mostly start with the greenery to set the stage and area where I want to cluster. Sometimes I many need to pull out the greenery a little more because the flowers cover them. Sometimes I tuck bits of greenery between the flowers. Sometimes I add more greenery if the cluster got larger than expected.
 
For me it is definitely the flowers first and then I work in the other stuff. They never come out with what was in my head at the beginning. LOL
 
I've always done flowers first, but now I kind of want to try it the other way. I've never thought of that before :)
 
I like to lay down a starting foundation so will place a bit of greenery down and then start building overtop of that. I almost always go back and reposition the greenery, dependent on the clustering size I've built. Sometimes I will then add more greenery or duplicate greenery to tuck in amongst the cluster layers. If I am going to use paint, overlays, etc, I will then place those under my foundation layer. I finish off with adding the smallest elements, buttons, sequins, tiny flowers, etc.
